FIRST AID BOX CHECKLIST-PLANT AND FACILITY SAFETY
DEVIATION CHECKLIST
1. The box is not completely stocked as per the list
2. The name of the First Aider and contact number are not displayed on the box
3. The location of the box is not indicated by symbolic signs
4. No book to record treatment from the box is available
5. The first aid box is in a poor state of repair
6. No Qualified First Aider is available in the area
7. The box cannot be removed in case of an emergency
8. The expiry date of the item has been exceeded
CORRECTIVE ACTION
1. Replenish the stock of the box
2. Display the name First Aider and contact number on the box
3. Indicate a box with symbolic signs
4. Provide a TREATMENT record book in the box
5. Repair or replace the first aid box
6. Nominate a person to be trained in First Aid
7. Ensure the box can be removed from its position with ease
8. Remove items that are beyond their expiry date
9. Provide MSDS HCS at the box
10. Provide an updated list of emergency numbers at the box
MINIMUM CONTENTS OF THE FIRST AID BOX
1. Wound Cleaner (500 ml)
2. Packet of swabs - for cleaning wounds
3. Sterilized gauze - (1 packet)
4. 1x pair of Forceps (for removing splinters)
5. 1x pair of Scissors (minimum size 100 mm)
6. 1 x card of Safety pins of various sizes
7. Cotton wool for padding (250 g)
8. 12 x Triangular bandages
9. 6 x Roller bandages (75mm x 5m) 106 x Roller bandages (100 mm x 5m)
10. 1x Roll of elastic adhesive (25mm x 3m)
11. 1x roll Antiallergenic adhesive (25mm x 3m)
12. 1 x Packet of adhesive dressing strips
13. 6 x large wound dressings (75mm x 100mm)
14. 6 x Extra-large wound dressings (150 mm x 200mm) 162 x Straight splints
15. Disposable gloves (2 pairs Large & 2 pairs Medium)
16. 2 x CPR mouthpieces or similar devices
17. 1x Chlorine solution (200 ml)
ADDITIONAL EXTRA (IF REQUIRED)
1. MSDS of hazardous chemicals used in the area
2. Names and telephone numbers of doctor and/or hospital
